ELIGIBILITY
Who Can Receive AFC Services?
Our Adult Family Care program is designed for eligible MassHealth members who need assistance with daily living activities and can safely live in a community setting.
MassHealth Members
Individuals must be enrolled in MassHealth to be eligible for AFC services.
Need for Daily Assistance
Must require help with daily living activities such as bathing, dressing, or medication management.
Community Living
Must be able to live safely in a family home or community setting.
Assessment Required
Eligibility is determined through a MassHealth assessment and care plan approval.
